I am currently undergoing a 360 evaluation as part of my ongoing professional development. The purpose of this process is to receive feedback on elements essential to my workplace effectiveness from diverse perspectives. Strengths and development areas identified from this process will be used to help define my training and development plan.

I am inviting you to complete a feedback survey on me because you are one of the people I work closely with and someone who can provide meaningful observations about my behavior.

You will receive an email from Eudara that contains instructions for accessing the online survey that will collect this feedback. It will take approximately 20-30 minutes for you to submit your feedback.

The Eudara 360 survey involves rating me on a number of items related to my effectiveness in my role and providing written comments regarding my strengths and development areas. Once the survey responses are gathered, a report is generated that combines your responses with those of others.  You will receive further instructions by email once the survey is launched.

I am looking forward to receiving this feedback as it can provide me with a very specific picture of what I am doing well, and where I need to focus my efforts to improve. I appreciate your willingness to help me with this process.
